We invite you to learn more about our services and meet the wonderful team at our new in-house pharmacy located at the Bellefontaine Community Health Center. This team feels strongly about connecting with patients and providing the same integrated approach to patient care that defines Community Health & Wellness Partners.
We are especially excited to make available a variety of new services, such as delivery services, either directly to your home within Champaign or Logan County or a more convenient CHWP location for pick up, including one of our school-based health centers. Additionally, we now offer pharmacy consultation, medication box refills and monitoring of chronic disease management such as heart disease, diabetes, and hypertension.
Of course, we will continue to provide the pharmacy services we always have, such as:
- Reviewing medications to ensure that patients are getting the most benefit from what they are taking.
- Helping patients find affordable medications.
- Ensuring that patients do not have drug interactions.
- Screening patients for depression.
- Performing pharmacogenomics testing.
- Conducting a cardiovascular disease risk assessment and initiating therapy as appropriate.
- Referring patients to diabetes education classes, ophthalmology, nutrition or social work services.
Patients may continue to reach the pharmacy via phone or the RxLocal app to manage all family prescriptions, request refills and set medicine reminders.
